Uses of medication:  Thiotepa is a alkylating agent, supplied generically under the brand Thiomed. This therapeutic drug is used:

  • To reduce the risk of graft rejection when used along with high-dose busulfan and cyclophosphamide as a preparative regimen for allogeneic hematopoietic progenitor (stem) cell transplantation (HSCT) for pediatric patients with class 3 beta-thalassemia.
  • For treatment of adenocarcinoma of the breast or ovary.
  • For controlling intracavitary effusions secondary to diffuse or localized neoplastic diseases of various serosal cavities.
  • For treatment of superficial papillary carcinoma of the urinary bladder.

Dosage and Side Effects

The recommended dose of Thiotepa for class 3 beta-thalassemia is two administrations of 5 mg/kg given intravenously (IV) approximately 12 hours apart on Day -6 before allogeneic HSCT along with high-dose busulfan and cyclophosphamide. 

The recommended dose of Thiotepa for the treatment of adenocarcinoma of the breast or ovary is 0.3 to 0.4 mg/kg administered intravenously.

The recommended dose of Thiotepa for the treatment of malignant effusions is 0.6 mg/kg to 0.8 mg/kg intracavitary.

The recommended dose of Thiotepa for the treatment of superficial papillary carcinoma of the urinary bladder is 60 mg in 30 to 60 mL of Sodium Chloride Injection into the bladder by catheter. 

The most common side effects of Thiotepa injection are hemorrhage, diarrhea, neutropenia, elevated alanine aminotransferase, anemia, thrombocytopenia, elevated aspartate aminotransferase, elevated bilirubin, mucositis, cytomegalovirus infection, hematuria, and rash.

Warning and Precautions

  • The consequence of therapy with high doses of Thiotepa along with other chemotherapy at the prescribed dose and schedule in the preparative regimen for class 3 beta- thalassemia is profound myelosuppression occurring in all patients. Avoid beginning the preparative regimen if a stem cell donor does not exist. Monitor complete blood counts (CBC), and provide supportive care for infections, anemia and thrombocytopenia until there is sufficient hematopoietic recovery.

Perform periodic complete blood counts (CBC) during the course of treatment. Provide supportive care for bleeding, infections, and symptomatic anemia.
  • Clinically significant hypersensitivity reactions, including anaphylaxis, may occur following the use of Thiotepa. In case of anaphylactic or other clinically significant allergic reactions, stop therapy, initiate suitable therapy, and monitor patients until signs and symptoms resolve.
  • Skin reactions linked with accidental exposure to Thiotepa treatment may develop. Wash the skin thoroughly with the help of soap and water in case Thiotepa solution contacts the skin. Flush mucous membranes in case of medicine contact with mucous membranes.
