Padcev (enfortumab vedotin-ejfv) is a targeted therapy that combines a Nectin-4-directed antibody with a microtubule inhibitor. It is prescribed

  • in combination with pembrolizumab to treat adults with locally advanced or metastatic urothelial cancer.
  • as a single agent for the treatment of adults with locally advanced or metastatic urothelial cancer. This includes patients who have previously been treated with a PD-1 or PD-L1 inhibitor and platinum-based chemotherapy or those who cannot receive cisplatin-based chemotherapy and have undergone one or more prior lines of therapy.

Posology & Administration: In combination with pembrolizumab, Padcev is typically administered intravenously over 30 minutes on Days 1 and 8 of a 21-day cycle. The recommended dose is 1.25 mg/kg, with a maximum of 125 mg for patients weighing 100 kg or more. Treatment continues until disease progression or unacceptable side effects occur.
For Padcev used alone, the recommended dosage is 1.25 mg/kg (up to a maximum of 125 mg for patients weighing 100 kg or more). It is given intravenously over 30 minutes on Days 1, 8, and 15 of a 28-day cycle until either disease progression or unacceptable side effects occur.
